Are you new to Cabot Physical Therapy?

We're here to make your experience with us as smooth as possible. If you're a new patient, welcome! Our team is ready to assist you on your journey to optimal health.

What to Bring:

  1. Referral (if applicable):
    - If you have a referral from your healthcare provider, please bring it with you. However, remember that a referral is not mandatory for your initial visit.
  2. Photo ID:
    - Please bring a valid photo ID, such as a driver's license or passport, to verify your identity.
  3. Completed Paperwork:
    - To save time during your visit, please complete any necessary paperwork provided to you in advance. You can download forms from our website or fill them out at our office.
  4. Loose-Fitted Clothing:
    - Wear comfortable, loose-fitted clothing that allows easy access to the areas our therapists may need to assess and treat.

What to Expect:

“You can do it with the persistent help of this team and hard work at home. July 2023, I had arthroscopic surgery of my left hip to repair a labral tear. Before surgery I had pain & catching with walking, gardening, horseback riding and cleaning house. After surgery, I was on a walker or crutches for 6-8 weeks. When I began PT, 2-3 times a week, I could barely lift my leg on my own. Starting with the very basics & sticking to it, I am now able to do walking, lifting and all my previous activities pain free.
